We're seeking compassionate, driven Case Managers to support residents in their journey toward independence, habilitation and successful community living. As a Case Manager, you will work directly with individuals transitioning from incarceration, helping them navigate key areas of life such as employment, healthcare (including mental health), personal finance, and essential life skills. This role is ideal for someone who believes in second chances and wants to make a meaningful impact every day.
Key Responsibilities:
Provide one-on-one support to residents based on their individual goals and needs. Create and manage personalized care plans that foster growth and stability. Connect residents with community resources, including medical, mental health, employment, and housing services. Support clients in building practical skills like budgeting, self-care, and job readiness. Track client progress, adjust plans as necessary, and document services in accordance with program guidelines. Complete regular reports to monitor outcomes and ensure compliance with program standards.
What We're Looking For:
Bachelor's degree (required). Experience in case management is preferred, but we welcome candidates with equivalent combinations of education and experience. Strong sense of empathy, good judgment, and the ability to set clear boundaries. Computer skills Report writing experience Valid driver's license and the ability to pass a background check.
