Job Description
What We Offer As a Case Manager BSW at Novant Health Huntersville Medical Center you will participate in the complex discharge planning process. Huntersville Medical Center i s an award winning 151 bed hospital in Huntersville, a family-friendly town in the Lake Norman area. This part-time position offers you more flexibility in your schedule while still offering part-time benefits. The schedule will rotate with 3 8 hour shifts and one full weekend every 6 weeks. You must be able to orient on a Monday - Friday basis for 4-8 weeks depending on experience. Novant Health offers you leadership support to help grow your career in the healthcare field. What You'll Do Participate in complex discharge planning process as a valued member of the healthcare team Collaborate with physicians and other members of the healthcare team to assess, plan, implement, and evaluate proactive, complex and safe discharge plans Collaborate with RN Case Managers for guidance with medically complex cases Serve as a resource for discharge planning and care coordination for psychosocially complex cases, working with MSW Case Managers as needed Responsible for facilitating coordination of care Maintain current knowledge of available community resources and post acute care options Accepts responsibility, authority and accountability for management of care in accordance with the current policies and procedures, mission, vision, and values of
