Qualified Professional - Intensive In-Home Services (IIH) Person County, NC | Paid Per Billable Contact Help Strengthen Families Where They Need It Most Youth Haven Services is seeking a compassionate and dedicated Qualified Professional (QP) to provide Intensive In-Home (IIH) services to children, adolescents, and their families throughout Person County, NC . This position is ideal for professionals who enjoy working independently, building meaningful relationships with families, and helping youth remain safely in their homes and communities. As a Qualified Professional, you will provide clinical oversight, case management, and skill-building services while working closely with licensed therapists and community partners to help families achieve lasting success. Position Summary The Qualified Professional provides Intensive In-Home case management and rehabilitative services for children and adolescents with behavioral and mental health needs. Services are delivered primarily in the client's home and community and are guided by each individual's Person-Centered Plan. This position works collaboratively with the licensed therapist, family members, schools, and other providers to ensure coordinated, quality care. This is a W-2 position and compensation is based on billable client contacts. Essential Responsibilities Complete client intakes, assessments, and Person-Centered Plans (PCPs). Submit initial authorizations and reauthorizations for Intensive In-Home services. Coordinate care with therapists, families, schools, physicians, and other community providers. Meet with clients and families in their homes, schools, and community settings to provide case management and skill-building services. Monitor client progress and assist families in achieving treatment goals identified in the Person-Centered Plan. Maintain accurate, timely, and compliant clinical documentation, including progress notes, service logs, treatment updates, and required agency paperwork. Complete
