Job Description

Greater Derry Community Health Services is seeking a compassionate, organized, and community‑minded Case Manager to join our dedicated team. This role supports residents across our service area by helping them navigate community resources, access general assistance programs, and receive the guidance they need during challenging times. About the Role The Case Manager provides direct support to residents seeking help with stress, financial difficulty, or navigating the human services system. CHS offers confidential, respectful, and prompt assistance, and our staff plays a key role in ensuring every individual feels supported. Responsibilities Meet with clients to assess needs and connect them with appropriate public and private community resources, including state benefits and other local programs they may be eligible. Support the General Assistance programs for Derry, Hampstead, Londonderry, and Plaistow. Assist residents seeking emergency aid, including rent, fuel, and utility support. Help applicants navigate documentation requirements and municipal guidelines. Maintain accurate case notes and track client progress to ensure timely, coordinated support. Schedule and conduct assistance appointments (Monday-Friday, 8:30am-4pm). Support front office operations, including answering phones and ordering supplies. Collaborate with the Program Manager to ensure smooth daily operations and strong community partnerships. Schedule In office, 30 hours per week Assistance appointments and office hours run Monday-Friday, 8:30am-4pm Compensation Hourly rate: to be determined based on candidate experience, education, etc.
