Case Manager

Some Are you passionate about making a difference in people's lives? We are seeking a Case Manager to join our team and provide essential support to individuals and families. What You'll Do:
  • Conduct assessments to identify clients' needs, strengths, and risks.
  • Develop and implement person-centered care plans tailored to individual goals.
  • Assist clients in applying for and maintaining state benefits.
  • Facilitate transitions to long-term service models.
  • Advocate for clients by coordinating with healthcare providers, and social service agencies.
  • Participate in key meetings, including ISAs, court hearings, and medical appointments.
  • Maintain accurate documentation and client records.
  • Monitor service effectiveness and identify areas for improvement.
What We're Looking For:
Education & Experience:
Bachelor's degree in a related field with at least 1 year of experience , OR Associate degree in a related field with 2 years of experience
Skills & Qualifications:
Strong interpersonal and communication skills Knowledge of person-centered care planning Ability to work effectively in a fast-paced environment Proficiency in documentation and record-keeping Familiarity with social service resources Why Join Us?
