The Urban League of Union County (ULUC) is a non-profit community service organization founded in 1944. We are one of 90 affiliates of the National Urban League. The Urban League supplies information on the minority population, conducts surveys, training programs, secures jobs, motivates youth, and provides essential services in education, housing, health, employment, and advocacy. Our mission is to assist African Americans and other underserved people in the achievement of social and economic equality. Programs operated included Housing Counseling (pre and post home purchase, budget/credit counseling, foreclosure, default, and delinquency counseling); Employment counseling, referral, and placement; Case management/re-entry counseling for those formerly incarcerated; and Emergency Assistance for rent/mortgage, utilities, and food. Summary The Case Manager is responsible for providing direct support to clients through intake assessments, service planning, resource coordination, and advocacy. This full-time role plays a critical part in helping individuals and families access resources to achieve stability and long-term success. The Case Manager will work collaboratively with other program staff and community partners to deliver high-quality, client-centered services. This position will primarily serve program participants throughout Elizabeth and Union County areas . The position requires regular local travel, reliable transportation, and flexibility to attend community events, correctional facility resource days, participant meetings, workshops, and other program activities. Key ResponsibilitiesClient Intake and Case Management Conduct comprehensive intake assessments and collect relevant personal, employment, educational, financial, housing, health, and social history information. Assess participants' strengths, barriers, immediate needs, and long-term goals. Develop and implement individualized service plans that support successful reentry, stability, and self-sufficiency. Provide ongoing case management, including regular participant contact, follow-up, progress monitoring, and service-plan updates. Build and maintain trusting, professional relationships that promote participant engagement and positive outcomes. Maintain an assigned caseload and ensure services are delivered in accordance with program requirements and established timelines. Resource Coordination and Referrals Connect participants with community resources, including employment and workforce development services, housing, healthcare, behavioral health services, education, transportation, identification documents, legal assistance, and public benefits. Assist participants with completing applications and navigating government agencies, community programs, and other service systems. Follow up on referrals to confirm that participants successfully accessed the recommended services. Develop and maintain relationships with community organizations and service providers throughout Union County and surrounding areas. Maintain current and accurate information regarding community resources, eligibility requirements, and referral procedures. Documentation and Reporting Accurately document all participant interactions, assessments, service plans, referrals, progress notes, and outcomes in the designated client management system. Maintain organized and confidential participant records in accordance with organizational policies, funder requirements, and applicable laws. Complete required documentation and reports accurately and within established deadlines. Collect and track participant data to support program evaluation, grant reporting, and continuous improvement. Immediately communicate significant participant concerns, incidents, or service barriers to the appropriate supervisor. Advocacy, Outreach, and Engagement Serve as a liaison between participants, community partners, correctional facilities, service providers, and other stakeholders. Advocate for participants' access to services, benefits, employment, housing, and other resources. Conduct outreach throughout the assigned service area to identify eligible participants and increase awareness of the Reconnections Reentry Program. Attend community events, prison and correctional facility resource days, workshops, partner meetings, and outreach activities. Support and facilitate workshops, group sessions, alumni meetings, and educational programming for participants and community members. Program Support Collaborate with program staff and community partners to improve case management practices and service delivery. Participate in staff meetings, case conferences, supervision sessions, and professional development activities. Support program evaluation efforts by collecting, reviewing, and analyzing participant and service-delivery data. Perform other program-related duties as assigned. Qualifications Bachelor's degree in Social Work, Human Services, Psychology, Sociology, Criminal Justice, or a related field required; master's degree preferred. At least one to two years of experience in case management, social services, reentry services, workforce development, or a related field. Experience working with formerly incarcerated individuals, justice-involved populations, or individuals facing multiple barriers to stability is strongly preferred. Strong interpersonal, written, and verbal communication skills. Ability to establish professional and supportive relationships with individuals from diverse backgrounds. Strong organizational, documentation, time-management, and follow-through skills. Ability to manage multiple cases while maintaining accuracy, professionalism, and confidentiality. Proficiency in Microsoft Office, including Word, Excel, PowerPoint, and Outlook, and familiarity with client management databases. Ability to work independently in the field and collaboratively as part of a program team. Bilingual proficiency in English and Spanish is preferred but not required. Travel and Scheduling Requirements Reliable transportation and a valid driver's license are required. Ability to travel throughout Union County. Ability to attend off-site meetings, outreach events, correctional facility resource days, and activities at other Urban League locations. Willingness to work a flexible part-time schedule, which may include occasional evenings, to accommodate participant intakes, follow-up meetings, workshops, alumni meetings, and community events.
