•Salary offer starts at $45,000; higher negotiation will be commensurate with experience and required duties.

•Case Manager II York Street Project's housing program seeks an experienced Case Manager with the skills and ability to enhance the quality of life for pregnant and/or parenting youth who experiencing homelessness or classified as run-away. The ideal candidate should be knowledgeable and skilled in working with vulnerable and at-risk populations, helping them stabilize their families and/or household and grow towards self-sufficency and independence. The tasks and responsibilities of this position include but are not limited to: Helping participants achieve wellness and autonomy through individual case management services Assisting with apartment hunting Assisting clients with the process of applying or obtaining benefits, entitlements, and basic needs Connecting participants with community-based programming to facilitate a continuum of service Linking participants with services and programs that provide specialized care (i.e. mental health, substance use, employment, child welfare, domestic violence etc.) Coordinating and providing care and service that is safe, timely, efficient, equitable, and client-centered Developing individual service plans and reviewing case progress Assessing participant's needs utilizing assessments and evaluations that inform service plans Attending and participating in interdisciplinary team meetings Attending and participating in community meetings Actively participating in the planning and execution of activities and events that improve the quality of life and stabilization of individuals and families with children Researching and keeping current with evidenced based treatment models, best practices, and community services and resources Assisting in the interviewing and admission of new participants Admitting and maintaining client information in national database Homeless Information Management Systems (HMIS) and internal database Apricot Assisting in the division, organization, and distribution of donations Following up with discharged clients to ensure interventions and services were successful at increasing long-term housing stability and improving overall quality of life Skills Needed Administrative abilities (good documentation) Practices good time management Organized Ability to work independently as well as part of a group Adapts well to transition and change Flexible Communicates well and professionally Pleasant attitude Good problem solver Shows initiative Creative and innovative Mission-driven Solution-focused Ability to adhere to professional standards as outlined by protocols, rules, regulations, and code of ethics. Qualifications Education Bachelor's Degree Experience Cultural respect and sensitivity working with ethnic and racial minorities and/or at-risk groups, especially young women and children and those living in poverty. Prior experience working with at-risk youth preferred. Personable Ability to communicate well, both orally and in writing. Proficient with Word and other Office Suite software Ability to work with ethnic and racial minority groups, especially women and children. Commitment to establishing a model of personal and institutional development that flows from values of inclusiveness and cooperation. Commitment to values of mutuality and collaboration. Valid driver's license preferred Bilingual/Spanish preferred.
