Pyramid House Foundation (PHF) is a New Jersey-based nonprofit organization dedicated to providing specialized housing and wraparound support services to underserved and marginalized communities. Guided by our three-pillar model - Housing, Healing, and Purpose - PHF delivers trauma-informed, person-centered, and culturally responsive services rooted in Housing First principles. Through our Muladhara Reentry Program, we provide transitional housing and reentry supports to formerly incarcerated adults. PHF is an approved provider agency with the NJ Department of Corrections through the NJLEAD program and a credentialed NJ FamilyCare Housing Supports Provider.
JOB SUMMARY
Pyramid House Foundation is seeking a dedicated and compassionate Reentry Case Manager to join our growing team. This position provides comprehensive case management and housing supports services to formerly incarcerated individuals, supporting their successful reintegration into the community. The Reentry Case Manager will also play a key role in PHF's NJ FamilyCare Housing Supports Program, delivering pre-tenancy and tenancy-sustaining services to Medicaid-enrolled members. KEY RESPONSIBILITIES
Reentry Case Management NJLEAD Program Tier 1
Pre-Release Assessment:
Conduct comprehensive reentry needs assessments to identify service needs including housing, counseling, education, employment readiness, and benefits. Develop personalized service plan based on assessment results, outlining housing needs and integration goals. Tier 2 -
Residency/ Community Phase:
Provide structured support and services to participants residing at Muladhara House or living in the community, helping them actively pursue goals outlined in their Housing Stabilization Plan. Collaborate with clients to address barriers to stability including employment, benefits access, mental health, and family reunification. Tier 3
Full Reentry:
Assist clients in transitioning to sustainable employment and permanent housing. Support clients in achieving independence and maintaining a sense of belonging and purpose. Housing Supports Services NJ FamilyCare Deliver pre-tenancy services including housing search assistance, rental application support, lease navigation, and landlord communication on behalf of Medicaid-enrolled members. Provide tenancy-sustaining services including individualized housing support plan development, budgeting and financial literacy coaching, lease compliance education, and eviction prevention intervention. Assist members in identifying and accessing community resources including rental assistance, utility supports, employment services, healthcare, and legal services. General Maintain accurate and timely documentation in the Homeless Management Information System (HMIS) and comply with all DMAHS reporting, monitoring, and claims submission requirements. Coordinate with Managed Care Organizations (MCOs), Support Coordinators, healthcare providers, and community partners to ensure integrated, person-centered care. Maintain a caseload of participants across NJLEAD and Housing Supports programs, conducting regular check-ins and progress reviews. Participate in team meetings, trainings, and professional development activities as required. Perform other duties as assigned by the Reentry Program Manager or
CEO. REQUIRED QUALIFICATIONS
Bachelor's degree in Social Work, Psychology, Criminal Justice, or a related field, or equivalent relevant experience. Lived experience of incarceration or reentry is valued and may be considered toward this requirement. Minimum of 2 years of experience in case management, reentry services, housing supports, or related fields. Lived experience navigating reentry, housing instability, or related systems may be considered toward this requirement. Strong understanding of the challenges faced by formerly incarcerated individuals and commitment to a trauma-informed, non-judgmental approach. Familiarity with pre-tenancy and tenancy-sustaining services, Housing First principles, and Medicaid-funded service delivery. Knowledge of or willingness to learn HMIS documentation and Medicaid claims submission processes. Excellent communication, organizational, and interpersonal skills. Ability to work independently and collaboratively as part of a small, mission-driven team. Valid driver's license and ability to travel throughout New Jersey.
PREFERRED QUALIFICATIONS
Master's degree in Social Work (MSW) or a related field; LMSW preferred. Experience working with Medicaid-enrolled populations and knowledge of NJ FamilyCare Housing Supports Program requirements. Familiarity with HMIS and experience with Medicaid documentation and billing. Experience with evidence-based practices in reentry, rehabilitation, and housing stability. Familiarity with
NJ DOC, NJLEAD
program, parole and probation systems, and local reentry resources. Bilingual (English/Spanish/Creole) strongly preferred. Lived experience with the criminal justice system is welcomed and valued.
