Job Overview The PLP Case Worker provides direct support to pregnant and parenting high school students through case management, resource coordination, group facilitation, and home visitation. This role is centered on reducing barriers to school attendance, promoting healthy child development, and supporting young parents in achieving academic and personal success. The Case Worker will work closely with students, school staff, childcare providers, and community partners to ensure a comprehensive, wraparound approach to care. Duties Case Management & Student Support

• Provide ongoing case management to pregnant and parenting students enrolled in the PLP program
• Conduct intake, assessments, and individualized service planning
• Maintain regular check-ins with students to monitor progress, attendance, and well-being
• Support students during pregnancy, postpartum transition, and return to school
• Conduct scheduled home visits to support student engagement, assess home environments, and strengthen family relationships
• Provide parent coaching and guidance on child development, routines, and bonding in the home setting
• Monitor safety, basic needs, and overall family well-being
• Support re-engagement for students with attendance barriers or extended absences
• Document home visits in accordance with program and compliance standards Experience Required
• Bilingual in English and Spanish (spoken and written)
• Bachelor's degree in Social Work, Psychology, Child Development, Youth Studies, or related field
• Experience working with youth, families, or vulnerable populations Preferred
• Experience working with pregnant or parenting teens
• Knowledge of community resources and public assistance systems
