1) Provides case management and counseling to clients at the Men's Emergency Shelter. This includes initial assessment, development of individual service plans, regular meetings with clients, arrangement for and attendance at meetings with important referral resources, staffing each case as needed with other St. Elizabeth Shelter staff members and outside professional resources and other services. 2) Case Manager is responsible for client's progress notes, case management notes and case manager weekly general notes. Case manager is also responsible for monthly summary of Case Management Services. 3) Assists in assessing clients for appropriateness for referral into other St. Elizabeth Shelter programs or outside agencies. 4) Assists other St. Elizabeth Shelter staff members in developing or revising agency standards for case management, client services, record keeping, discharge planning, and other program-related functions. 5) Enters client information into County database. 6) Meets, as needed, with staff of other agencies to develop or enhance agreements and protocol for referrals and services to St. Elizabeth Shelter clients. 7) Attends regular staffing meetings with the rest of staff to review specific clients, overall protocols, issues, and areas needing improvement. Participates in corrective action plans for individual clients and agency protocols. 8) Performs other tasks as required to provide effective case management services to St. Elizabeth Shelter clients. 9) Shares on-call duties with other members of the case management team in responding to emergencies. 10) Provides mentoring and assistance to the part-time and intern case managers. Requirements Credentials/Qualifications Bachelor's degree in social work, counseling, psychology, or related field preferred, or equivalent work experience. Ability to lift 25 pounds. Experience and Skills Spanish speaking preferred Experience working with homeless individuals and/or families preferred Willing and able to work with diverse populations Effective communication skills Computer proficient Knowledge of the unique cultural heritage of northern New Mexico Other Valid driver's license and proof of insurance Willing and able to work irregular hours and have a flexible work schedule
