A DAY IN THE LIFE

As a Case Manager, every day is a little different, but your goal is always the same. Discerning the disabilities and accompanied needs of our participants as well as helping them achieve their goals is at the top of your priority list. You carefully make a plan for each participant and modify it annually or when there is a significant medical or social change. With close attention to detail and professionalism, you communicate with other individuals and entities to ensure seamless coordination between physical, behavioral, and support services. In addition to managing cases, you are consistently developing and maintaining professional working relationships with participants and their families.
