POSITION SUMMARY
Under the direction of the Director of Programs, the Case Manager will provide social services assistance to improve the stability of low-income families and to maximize their family well-being which leads to self-sufficiency. This position will assist clients in removing barriers to housing and employment. This position will assist with referrals for counseling, community resources, government programs, and other resources for families and children. The ideal candidate will have the following:
ESSENTIAL DUTIES AND RESPONSIBILITIES
  • Must maintain case history records and prepare reports.
  • Must interview clients individually, families or in groups to assess their situations. capabilities, and problems to determine what services are required to meet their needs.
  • Must develop and review goal plans in consultation with clients and perform follow-ups assessing the quantity and quality of services provided.
  • Follow-up 30, 60, 90 days or greater when needed.
  • Must be able to give full attention to what other people are saying and take time to understand their perspectives.
  • Must be able to document client information into the case management system in a timely manner.
  • Must use good judgement and make sound decisions.
  • Must be able to de-escalate situations with clients.
  • Must be able to give counsel to Case Managers.
  • Must be able to respond to messaging in a timely manner via phone, email, etc.
QUALIFICATION REQUIREMENTS

EDUCATION AND/OR EXPERIENCE
Two years' experience in a Case Management support position. Must have at least a minimum 3 years' experience working face to face with the general public.
Language Skills:
Knowledge of the structure and content of the English language including the meaning and spelling of words, rules of composition, and grammar. Must have proficiency in speaking, reading and writing Spanish is preferred. This position must have a good understanding of diversity and cultural awareness
Mathematical Skills :
Ability to apply basic mathematics in order to accurately monitor budgets.
Reasoning Ability:
Ability to solve practical problems and deal with a variety of variables in situations where limited standardization exists. Ability to use logic and reasoning to identify the strengths and weaknesses of alternative solutions, conclusions or approaches to problems. Must be able to consider relative costs and benefits of potential actions and could choose the most appropriate solution.
OTHER SKILLS AND ABILITIES
Proficiency in working with specific computer software such as spreadsheets, word processing, and database management is required. Knowledge of human resource systems and the Internet are also required in order to perform this job successfully. Proficiency in the use of computers and all common office machines is required. Adequate typing skills are necessary for this position. Must be able to work sensitively and effectively with individuals of diverse educational, socio-economic and cultural background. Strong organizational skills.
OTHER REQUIREMENTS
If an offer is extended, the candidate will be required to successfully complete a criminal background check as well as pre-employment drug screening.
CERTIFICATES, LICENSES, AND REGISTRATIONS
Must be trained on the principles and procedures of Results Oriented Management and Accountability (ROMA).
