Case Manager

We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will play a crucial role in supporting individuals facing various challenges, including addiction, behavioral health issues, and social welfare needs. This position requires strong crisis intervention skills and the ability to work collaboratively with clients to develop effective support plans. You will be responsible for guiding clients through their unique journeys toward recovery and long-term self-sufficiency.

Responsibilities Assessment & Planning:

Conduct comprehensive assessments of clients' needs and develop individualized service plans.

Crisis Management:

Provide immediate crisis intervention and management for clients in urgent situations.

Specialized Support:

Offer addiction counseling and resources for individuals struggling with substance use disorders.

Advocacy & Navigation:

Assist clients in navigating Section 8 housing applications, child welfare services, and Medicare processes.

Empowerment:

Utilize motivational interviewing techniques to empower clients in their recovery and facilitate workforce development programs to enhance employability skills.

Collaboration:

Partner with healthcare providers, social services, and community resources to ensure holistic care.

Leadership:

Supervise and mentor junior staff or interns as needed, fostering a supportive learning environment.

Documentation:

Maintain accurate, timely documentation of client progress and case notes in compliance with organizational policies.

Qualifications Education:

Bachelor's degree in Social Work, Psychology, or a related field (Master's preferred). High School Diploma combined with significant relevant experience will also be accepted.

Experience:

Proven experience in crisis intervention and behavioral health settings.

Knowledge:

Familiarity with addiction counseling techniques, child welfare systems, and Section 8 housing regulations.

Interpersonal Skills:

Excellent verbal and written communication skills with a proven ability to build rapport with diverse populations.

Teamwork:

Ability to work independently while remaining a proactive, reliable member of a collaborative team.

Compensation & Details Pay:

$16.00 - $17.50 per hour
