Responsibilities:

Assist with the integration of individuals into our senior living facility. Ensure or provide therapeutic interventions to assist residents in coping with their transition and adjustment to the facility, including addressing their social, emotional, and psychological needs while following DOH guidelines. Act as a liaison between residents, families, and other healthcare entities. Assist with quality assurance reviews and maintain accurate, up-to-date files and case management notes. Education & Experience:

Bachelor's degree in a human services field and one year of full-time work experience in a related field; or Associate's degree in a human services field and three years of full-time work experience in a related field.
