Salary:
$22.00-$24.00
Hourly Minimum Qualifications:
Associates Degree in Health/Human Services. Bachelors Degree preferred. Valid NYS Drivers' License. Pursuit of QHP status is encouraged. Required CASAC T within 30 days of hire.
Job Summary:
In accordance with the organization's mission, vision and values, the Supportive Living Counselor/Case Manager is responsible for case management services for individuals who have been accessed as needing addictions services.
The following duties will apply:
Duties:
1. Responsible for the development and implementation of individual client service plans, ensuring that plans are initiated; provide follow-up and modification as necessary. Assure that record keeping is in compliance with Agency/OASAS regulations and policy & procedures as well as case management duties. 2. Demonstrates teamwork and an ability to work in a team environment with coworkers and supervisors. Demonstrates the ability to be flexible and cooperative. 3. Responsible for independent completion of case management duties. 4. Facilitates community meetings and group support sessions for residents. Designs, implements and coordinates groups based on the needs of the clients. 5. Responsible for providing counseling to residents in order to assist clients in the development of appropriate interpersonal, problem-solving and decision making skills. 6. Responds appropriately to crisis/emergency situations in order to ensure/secure the safety/well-being of the clients. 7. Responsible for completion of discharge activities according to policies, practices and procedures. 8. Assures all documentation is completed in a timely and accurate manner in compliance with agency/OASAS regulations, practices, policies & procedures. 9. Demonstrates professionalism by providing quality service and care with respect and dignity for the clients and as a representative of
FLACRA. 10.
Participate in general, staff, in-service and supervisory meetings held by the Agency. 11. Perform other duties and functions as directed. Other Skills/Knowledge and Experience 1. Excellent written and verbal communication skills. 2. Demonstrated ability to communicate effectively and work cooperatively with culturally diverse persons, staff and community service providers. 3. Knowledge of local behavioral health system. 4. Ability to multi-task, have good problem solving and time management skills and the ability to remain calm in crisis. 5. Proficient in E-record.
