Case Manager

At Harmonia Collaborative Care, our Health Homes team plays a vital role in helping individuals navigate complex systems with dignity and support. We're seeking a Case Manager who is organized, compassionate, and motivated to make a real difference in the lives of those we serve. This position requires a valid NYS drivers license and reliable vehicle. Travel within the community is required. This role is ideal for someone who values relationship-based work and meaningful community impact.
