The Home-Based Crisis Intervention (HBCI) program is an intensive, short-term 4- to 6-week in-home/community program designed to prevent out of home placement, including psychiatric hospitalization, residential placement or emergency department visits. Services are available to families residing in Nassau County. HBCI counselors are on call 24 hours a day, 7 days a week, to help families manage crisis and provide support. The Home Base Crisis Intervention Program at South Shore Child Guidance is looking for a full-time caseworker to service children, adolescents and families in Nassau County. This position is 35 hours/week, with a flexible schedule.
Job Responsibilities:
Field/Home Visits Provide crisis counseling to a child at risk of psychiatric hospitalization and his/her and families. Obtain intake information and relate it to the present behavioral and emotional issues of the child. Prepare Crisis Safety Plans, Action Plan, and maintain appropriate documentation in charts. Coordinate services with current treatment providers, school staff, etc. when applicable. Provide referrals to ongoing treatment and supports services.
