Case Manager Chautauqua County Department of Mental Hygiene & Social Services Dunkirk, NY Full-Time | Excellent Benefits | Public Service Career Opportunity Make a Difference in Your Community The Chautauqua County Department of Mental Hygiene & Social Services is seeking qualified applicants for a full-time Case Manager position in our Dunkirk office. The Case Manager helps clients identify barriers to self-sufficiency, develop personalized plans for success, connect with community resources, and achieve employment and life goals. As a Case Manager, you will: Assess client strengths, needs, and barriers through interviews and ongoing case management Develop and monitor individualized employability and self-sufficiency plans Connect clients with community resources, educational opportunities, job training programs, childcare assistance, financial aid, and other supportive services Coordinate services and advocate on behalf of clients with community agencies and service providers Monitor client progress toward employment and self-sufficiency goals Maintain detailed case records, documentation, and reports Conduct home visits, worksite visits, and community-based appointments Provide vocational guidance, job search assistance, mock interviews, and work-readiness coaching Facilitate workshops, orientation sessions, support groups, and recruitment activities Assist clients experiencing emergency situations and other barriers to success Serve as a liaison for clients requiring domestic violence support services Help arrange supportive services such as transportation, housing resources, and childcare assistance Work collaboratively with Transitional Assistance staff and other service providers Successful candidates will demonstrate: Strong interpersonal and communication skills Compassion and a commitment to helping others succeed Ability to motivate, coach, and support diverse populations Strong organizational and case documentation skills Problem-solving and critical-thinking abilities Ability to work independently and as part of a team Comfort working both in the office and in the community Minimum Qualifications Candidates must possess: Successful completion of 60 semester credit hours from a regionally accredited or New York State registered college or university; and Two (2) years of experience involving direct client contact in the delivery of services within a human services agency or program. Additional Requirements Must be able to meet transportation requirements necessary to conduct fieldwork assignments at the time of appointment and throughout employment. Candidates must not have been convicted of an alcohol- or drug-related driving offense, nor have refused a Blood Alcohol Content (BAC) or chemical test, within three (3) years prior to examination, certification, or appointment. Appointment Information This position is being filled on a provisional basis pending the administration and establishment of an eligible list from a future Civil Service examination for this title.
