Case Managers needed at our community residences for families. Per Diem coverage shifts are available for all days of the week as needed 4PM-12AM

AND 8AM-4PM

nights, weekends, and holidays. Locations in Medford and Ronkonkoma. Part-time available after successful completion of Per Diem shift coverage.

MUST BE WILLING TO WORK HOLIDAYS WHICH INCLUDES A FINANCIAL INCENTIVE.

Holidays needed are Thanksgiving, Christmas Eve, Christmas Day, New Years Eve, and New Years Day. Case management of public assistance recipients/families. Progress noting, goal plans, intake assessments, rule compliance, face to face meetings in person/over the phone, and supportive counseling. Looking for applicants who are ready to work in an environment dealing with challenged families who experience poverty and mental illness.

Qualifications:

Experience in human services and social work. Must be fingerprinted through the Suffolk County Department of Social Services. Fingerprinting will be paid by us. Paid time off is included for employees on schedule for more than 24 hours per week after successfully completing a 90 day orientation. Must have good boundaries, reliable transportation and must be dependable during bad weather/ on holidays. Supportive management team looking forward to working with reliable individuals.
