Salary:

$55,000 annually

Position Summary:

Supported Housing Case Manager helps clients maintain stable housing, strengthen independent living skills, and access essential community resources and acts as both an advocate and a connector, ensuring residents receive the individualized services they need. Responsibilities Deliver case management services including service planning, medication monitoring, and referrals to clinical, vocational, and community-based programs. Provide individual counseling and support, ensuring all documentation is timely and accurate. Assist clients in maintaining housing stability and addressing maintenance or landlord issues. Conduct regular site visits and monthly fire safety inspections. Support clients with budgeting, daily living skills, and community engagement. Participate in case conferences, supervision, and weekly staff meetings. Handle crisis intervention and provide 24/7 on-call coverage as scheduled. Qualifications Bachelor's Degree in Psychology, Social Work, Rehabilitation, or a related field. Minimum of 2 years' experience working with individuals with mental health conditions or in a supportive housing setting.

Pay:

$55,000.00 per year
