Job Description

Overview Join our dedicated team as a Residential Addictions Case Manager and become a vital part of transforming lives through compassionate, comprehensive addiction recovery support. In this role, you will provide client-centered case management services within a residential setting, guiding individuals through their recovery journey with expertise, empathy, and proactive coordination. Your work will help clients access essential resources, develop coping skills, and achieve lasting sobriety while ensuring safety and adherence to healthcare standards. This is an exciting opportunity to make a meaningful impact! Residential CASAC Case Manager -
Men's Stabilization/Rehabilitation Residence Primary Purpose:
Residential Addictions Case Manager will provide counseling services to clients in accordance with OASAS and Agency rules and regulations in a stabilization/rehabilitation residential setting.
Responsibilities :
1. Participates in screening of clients, gathering of necessary admission information/data, and observation of physical, mental, and emotional status of admitted clients. 2. Works cooperatively with the program manager to meet the treatment needs of each client. Must assume a rotating caseload. 3. Provides individual and group counseling sessions to residents, this includes motivation, support and encouragement for client goals. 4. Collects urine drugs screens and breathalyzer testing for clients. Prepares all paperwork related to routine and random urine and drug screens. 5. Monitors clients for medical, mental health, safety and security issues. Monitors client medication schedules, following program procedure and protocols. 6. Prepares all treatment plans, and discharge plans. Assumes responsibility for managing and reporting incidents. 7. Participates in weekly Case Conferences and resident meetings including team meetings. Attends required staff meetings and trainings. 8. Make calls to local ambulance service, fire department, police, or CDPC, as needed. 9. Performs other duties as assigned by program manager.
Education Requirements :
Candidate must have a minimum of High School Diploma or GED.
License Requirements :
Candidate must be credentialed as a CASAC-T, CASAC-P or CASAC and have a valid NYS Driver's License.
Experience Requirements :
Preferably two (2) years' experience in a chemical dependency program.
Schedule:

Pay:
$22.00 - $28.00 per hour
