As the STEPH Case Manager, you will play a pivotal role in individuals' lives, making a significant impact through: Accept and process referrals for individuals and families experiencing homelessness. Complete all required documentation and verifications for each household referred, including assessments and necessary release forms. Summarize assessment findings, and develop a personalized service plan with specific goals and referrals to local resources. Determine the frequency of contact with each household based on the completed assessment, ensuring contact-either in person or by phone-at least once every two weeks. rovide support to overcome barriers to achieving the goals in the service plan, which may include financial assistance and transportation to ensure households can access essential services. Deliver direct services to households referred to the Program. These services include advocacy, coordinating resources, referrals, transportation, budgeting, employment readiness support, and housing-related financial assistance. Services may be provided in various settings, such as the household s home, community locations, other service providers' offices, or Catholic Charities offices. STEPH Case Manager Salary:
$ 22.00 / hourSTEPH Case Manager Schedule:
Monday- Wednesday, and Friday 8:00am
- 4:00pm; Thursdays 8:00am
- 6:30pm (35hours / week)
Qualifications:
