A Case Manager coordinates delivery of a range of social and health services to individuals and families. Assess client needs and work with agencies and institutions ensure clients get the help they need. Monitor client and work with other service providers as necessary to address client needs.

Job Description

Are you passionate about advocating for individuals who may not be able to advocate for themselves? Join our team as a Case Manager with the Community Housing Assistance Program (CHAP) and help individuals experiencing homelessness access resources, stabilize their lives, and move toward permanent housing.

Key Responsibilities:
Build supportive relationships with participants experiencing homelessness.

Provide supportive counseling and daily living skills training.

Connect participants to community resources and assist with discharge planning.

Provide crisis intervention and assist with meeting basic needs.

Conduct home visits and provide transportation when needed.

Maintain documentation and case records.

Promote a culture of inclusion and belonging.
Qualifications:
Associate or Bachelors degree in Human Services, Social Work, or a related field preferred (equivalent experience considered).Experience working with diverse or high-needs populations preferred.

Valid NYS drivers license and access to a reliable vehicle required.
