position may be eligible for a $1,250 sign on bonusAbout the RoleUnity Houses Transitional Apartment Services Program (TAS) is seeking a compassionate and motivated Case Manager to join our team! This position supports individuals living with mental illness on their journey toward independence and community integration. Apply today and bring your positive energy to Unity House! Key ResponsibilitiesProvide supportive services to program participants with mental illness, ensuring compassion, dignity, and respect.
Assist participants in developing daily living skills and achieving personal goals.
Maintain documentation and participant charts in compliance with Office of Mental Health (OMH) standards.
Communicate regularly with healthcare and social service providers to coordinate care.
Assist with light to moderate cleaning in participant apartments as needed.
Promote an environment of inclusion, belonging, and empowerment for all participants.

Qualifications:
Education:
High School Diploma or equivalent required.Experience:
Minimum of one (1) year of qualifying experience in human services; experience working with diverse populations strongly preferred.Licenses:
Must have reliable transportation and a valid NYS drivers license.Language Skills:
