Job Description
Job Overview We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will play a crucial role in supporting individuals and families by providing guidance, resources, and advocacy. This position requires a strong understanding of social services, behavioral health, and crisis management to effectively assist clients in navigating their challenges and achieving their goals. Responsibilities Conduct comprehensive assessments to determine client needs and develop personalized service plans. Provide ongoing support and case management services, including regular follow-ups to monitor progress. Facilitate access to community resources such as housing assistance (including Section 8), addiction counseling, and workforce development programs. Utilize motivational interviewing techniques to empower clients in making positive life changes. Offer crisis intervention and management services to clients facing immediate challenges. Collaborate with multidisciplinary teams, including social workers and mental health professionals, to coordinate care. Mentor clients by providing guidance in areas such as childcare and personal development. Maintain accurate documentation of client interactions, progress notes, and service plans. Experience Previous experience in case management or a similar role is highly desirable. Familiarity with addiction counseling, behavioral health services, and crisis intervention strategies is beneficial. Strong interpersonal skills with the ability to build rapport with diverse populations. Excellent organizational skills and attention to detail for effective documentation and reporting. Ability to work independently as well as collaboratively within a team environment.
