  • Bilingual Reports to: Deputy Director Yonkers Community Action Program, Inc.
(YCAP) is seeking a dedicated and compassionate Bilingual Case Manager (English/Spanish) to join our Community Services team. This position plays a critical role in helping low-income individuals and families achieve economic stability and self-sufficiency by connecting them with emergency assistance, supportive services, and community resources. Position Summary The Bilingual Case Manager is responsible for providing comprehensive case management, assessment, advocacy, referral, and supportive services to eligible individuals and families through the Community Services Block Grant (CSBG) Program. The position assists residents facing economic hardships by helping them access resources such as: Rental Assistance Utility Assistance Food Pantry Services Clothing Vouchers Furniture Assistance Emergency Financial Assistance Housing Stabilization Services Community Resource Referrals The Case Manager works closely with participants to identify barriers, develop self-sufficiency goals, and connect households to resources that improve long-term stability and quality of life. Bilingual Requirement Fluency in both English and Spanish is

Candidates must be able to effectively read, write, speak, and communicate in both languages. Responsibilities include conducting assessments, providing case management services, translating program information, and assisting Spanish-speaking participants throughout the intake and service delivery process. Responsibilities Case Management & Client Services Conduct intake interviews and eligibility screenings. Complete participant assessments and service plans. Provide ongoing case management and support services. Data collection and reporting activities (CAP60). Develop Family Self-Sufficiency Plans and monitor participant progress. Provide crisis intervention and advocacy services. Maintain regular communication with participants and partner agencies. Conduct follow-up activities and monitor outcomes. Emergency Assistance Programs Coordinate and administer services including: Rental Assistance Utility Assistance Food Pantry Services Clothing Assistance Furniture Assistance Emergency Financial Assistance Assist participants in securing resources needed to maintain housing stability and address immediate needs. Resource Coordination & Referrals Connect participants to community resources including: Employment Services Education and Training Programs Childcare Assistance Healthcare Services Mental Health Services Transportation Assistance Housing Programs Legal Services Veteran Services Senior Services Develop and maintain collaborative relationships with community partners and service providers. Community Outreach Participate in community events, resource fairs, and outreach activities. Conduct outreach in both English-speaking and Spanish-speaking communities. Promote awareness of agency services and available resources. Represent YCAP professionally within the community. Documentation & Compliance Maintain accurate case records and documentation. Complete case notes, assessments, and service plans. Track participant outcomes and program performance measures. Ensure compliance with CSBG and agency policies. Assist with audits, monitoring, and reporting requirements. Qualifications Education Bachelor's Degree in Social Work, Human Services, Sociology, Psychology, Public Administration, Community Development, or a related field preferred. Associate Degree with substantial related experience may be considered. Experience Minimum of two (2) years of experience in case management, social services, housing programs, community services, or human services preferred. Experience serving low-income individuals and families strongly preferred. Community Action Agency or CSBG experience is a plus. Knowledge, Skills & Abilities Fluency in English and Spanish (written and verbal) required. Strong interviewing and assessment skills. Knowledge of public benefits and community resources. Excellent communication and interpersonal skills. Strong organizational and documentation abilities. Ability to work effectively with diverse populations. Proficiency in Microsoft Office and case management systems. Commitment to maintaining confidentiality and professionalism. Licenses Valid New York State Driver's License required. Reliable transportation preferred.
