Invisible Wounds Position Title:
Case Manager Reports to: Deputy Director Position Summary The CSBG Case Manager provides comprehensive case management services to low-income individuals and families throughout Westchester County. This position helps participants overcome barriers to self-sufficiency by conducting assessments, developing service plans, coordinating assistance programs, providing advocacy, and connecting clients to critical community resources. The Case Manager administers a variety of support services, including rental assistance, utility assistance, food assistance, clothing and furniture support, emergency financial assistance, housing stabilization services, and referrals to community-based programs. Key Responsibilities Case Management & Client Support Conduct client intake, eligibility determination, and needs assessments. Data collection and reporting activities (CAP60). Develop individualized service and self-sufficiency plans. Provide ongoing case management, support, and follow-up services. Advocate for participants and coordinate access to needed resources. Maintain communication with participants, landlords, utility providers, and community partners. Provide crisis intervention and problem-solving support. Emergency Assistance Programs Administer and coordinate: Rental Assistance Utility Assistance Food Pantry and Food Resources Clothing Assistance Furniture and Household Goods Assistance Emergency Financial Assistance Housing Stabilization Assist households experiencing housing instability or risk of homelessness. Develop housing stabilization plans. Coordinate referrals to housing and supportive service programs. Work collaboratively with landlords and housing providers. Monitor participant housing outcomes and progress. Community Resources & Referrals Connect participants to: Employment and workforce development programs Education and training opportunities Healthcare services Childcare resources Mental health services Transportation assistance Senior and veteran services Domestic violence support services Legal assistance programs Documentation & Compliance Maintain accurate client records and case notes. Enter data into agency databases and case management systems. Track outcomes and performance measures. Ensure compliance with CSBG requirements and agency policies. Assist with reporting, audits, monitoring visits, and program reviews. Outreach & Community Engagement Participate in community outreach events and resource fairs. Build relationships with referral partners and stakeholders. Promote program awareness throughout underserved communities. Qualifications Education Bachelor's degree in Social Work, Human Services, Psychology, Sociology, Community Development, Public Administration, or a related field preferred. Associate degree with significant relevant experience may be considered. Experience Minimum of two (2) years of experience in case management, human services, social services, housing assistance, or community services preferred. Experience working with low-income individuals and families strongly preferred. Community Action Agency or public assistance program experience is a plus. Skills & Knowledge Strong assessment, interviewing, and case management skills. Knowledge of community resources and public benefits programs. Excellent communication and interpersonal skills. Strong organizational and documentation abilities. Ability to manage multiple priorities and deadlines. Proficiency with Microsoft Office and electronic case management systems. Commitment to maintaining confidentiality and professional boundaries. Preferred Qualifications Case Management Certification or related human services certification. Knowledge of Community Services Block Grant (CSBG) programs and anti-poverty initiatives. Experience with housing stability or emergency assistance programs. License Requirements Valid New York State Driver's License required. Reliable transportation preferred.
