Job Description

Overview:
On Demand Counseling is seeking multiple compassionate, organized, and motivated Case Managers to join our Austintown team. This position plays a vital role in helping individuals with substance use and mental health disorders overcome barriers to treatment and recovery by connecting them with the resources and support they need to succeed.
Shifts/Schedule:
Full-time Weekday and weekend schedules available. Weekend-only schedules may also be available.
Responsibilities:
Assist clients with obtaining and maintaining basic needs, including food assistance, Medicaid, housing, identification documents, transportation, and other community resources. Coordinate referrals to medical, behavioral health, vocational, and social service providers. Collaborate with counselors and the multidisciplinary treatment team to support client progress. Assist with discharge planning and linkage to ongoing community supports. Advocate for clients and help them navigate complex systems and services. Maintain timely, accurate documentation in the electronic health record. Monitor client progress toward case management goals and follow up on identified needs. Develop positive working relationships with community agencies and referral partners.
Qualifications:
Must be at least 18 years of age. Successful completion of BCI/FBI background check and pre-employment drug screen. Minimum of a high school diploma or equivalent Minimum of one year of case management experience required. CDCA credential preferred; candidates willing to obtain the credential are encouraged to apply. Credentialing support is available for the right candidate.
Skills/Experience:
Strong communication, organization, and problem-solving skills. Ability to work independently while collaborating effectively within a multidisciplinary team. Benefits Competitive wages.
