Our mission is to provide quality and accessible primary healthcare services through community responsive approaches that address financial, geographic, and other barriers to care for residents of northern Hamilton County and surrounding areas. THCC is proudly recognized as a Level 3 Patient Centered Medical Home (PCMH), the highest level of recognition attainable for quality care. The HealthCare Connection is seeking a Case Manager to support our high-risk and complex patient population. In this role, you will provide case management and outreach services, helping patients overcome barriers related to social determinants of health and guiding them toward improved health outcomes and quality of life. The Case Manager will assess patient needs, develop individualized care plans, and connect patients and families with essential community resources. This role requires strong collaboration with an interdisciplinary care team to ensure coordinated, patient-centered care.Key Responsibilities :
Identify at-risk patients experiencing challenges related to social determinants of health using screening tools (e.g., PRAPARE) Conduct comprehensive assessments of patient and family needs, including physical, psychosocial, educational, and resource-related factors Assist in coordinating care plans in collaboration with healthcare providers and interdisciplinary teams Develop and facilitate patient-centered care plans in alignment with Primary Care Providers and Behavioral Health Providers Ensure effective communication and coordination to avoid duplication of services Provide referrals and support for mental health and substance use treatment programs Advocate for patients by assisting with problem-solving, education, and self-management coaching Schedule and ensure completion of follow-up appointments, screenings, and referrals Maintain accurate documentation and patient tracking systems, including services provided and outcomes Build and maintain relationships with community agencies and external resources Serve as a resource and liaison for patients, families, and the healthcare teamQualifications :
Education:
Bachelor's degree in Social Work, Sociology, Psychology, or a related field requiredExperience & Skills:
