Full Job Description Job Title:
Senior Services Case Manager Reports to: Programs Director The Senior Services Case Manager will provide case management support for residents at our homeless shelter and assist with various senior-focused programs, including the Senior Day Program and Kinship Program. Key Responsibilities Case Management for Homeless Shelter Guests
  • Conduct assessments to identify the needs of senior shelter residents and develop individualized service plans.
  • Connect residents with housing resources, medical care, mental health services, and other supportive programs.
  • Provide crisis intervention and support in navigating social services.
  • Advocate for clients with external agencies, ensuring access to benefits and housing opportunities.
  • Monitor and document progress, adjusting service plans as needed. Support for Senior Day & Kinship Programs
  • Assist in planning and coordinating activities for the Senior Day Program, ensuring an engaging and supportive environment.
  • Provide resources and case management support for kinship caregivers, helping them access financial aid, healthcare, and legal assistance.
  • Facilitate group discussions, workshops, and educational sessions for program participants.
  • Collaborate with community partners to enhance program offerings and expand available resources.
  • Maintain accurate records of resident participation in seeking independent housing.
  • Complete 30 and 60-day follow-up visits with former clients in the senior guest house.
  • Assist with compiling reports and documentation for senior programs.
  • Answer phones and provide administrative support to the Senior Programs Director.
  • Collaborate with other staff members to ensure smooth operations within the Guest House.
Complete and submit a weekly summary of duties each week. Complete other duties as assigned.
Pay:
$20.00 per hour
