The Case Manager
Lead at the Community Action Agency of Oklahoma is responsible for overseeing and guiding the case management team in delivering comprehensive support services to clients. Key Responsibilities:
Lead and supervise a team of case managers, providing guidance, mentorship, and support to ensure high-quality service delivery.
Develop and implement case management strategies that align with the agency's goals and objectives.
Coordinate with community partners and stakeholders to enhance resource availability and service accessibility for clients.
Conduct regular assessments of client needs and progress, ensuring that case plans are tailored to individual circumstances and updated as necessary.
Facilitate training and professional development opportunities for the case management team to enhance their skills and knowledge.
Monitor and evaluate program outcomes, preparing reports and recommendations for continuous improvement.
Ensure compliance with agency policies, procedures, and regulatory requirements in all case management activities.
Foster a positive and collaborative work environment that encourages teamwork and innovation.
Qualifications Required Education:
Bachelor's degree in Social Work, Psychology, Sociology, or a related field.
Required Experience:
Minimum of 5 years of experience in case management or a related field.
Proven track record of leading a team or managing projects within a social services environment.
Experience working with diverse populations and understanding of cultural competency.
Familiarity with community resources and services in Oklahoma.
Required Skills and Abilities:
Strong leadership and team management skills, with the ability to motivate and guide staff effectively.
Excellent communication skills, both verbal and written, with the ability to interact with clients, colleagues, and community partners professionally.
Ability to assess client needs, develop service plans, and monitor progress.
Proficient in using case management software and Microsoft Office Suite.
Strong organizational skills with the ability to manage multiple priorities and deadlines.
Ability to work independently and as part of a team in a fast-paced environment.
Demonstrated problem-solving skills and the ability to make sound decisions under pressure.
