Case Manager Heroes Hope Tulsa, OK Job Details Full-time $20 - $27 an hour 1 day ago Benefits Paid training Paid time off Qualifications Crisis intervention Computer operation Computer literacy Driver's License Bachelor's degree Case management Full Job Description Heroes Hope is seeking a compassionate, organized, and mission-driven Case Manager to support veterans experiencing homelessness or housing instability. This role serves as a critical point of contact for clients, helping them navigate resources, develop individualized care plans, and move toward long-term stability and independence. The ideal candidate is relational, detail-oriented, and passionate about serving those who have served our country. Key Responsibilities Assessment & Care Planning Conduct comprehensive client assessments to identify immediate and long-term needs Develop individualized service and care plans based on client goals Assist clients in establishing achievable objectives related to housing, employment, healthcare, recovery, and stability Service Coordination Connect clients with medical, mental health, housing, employment, and community resources Coordinate referrals and appointments with partner agencies and providers Collaborate with community organizations, landlords, VA programs, and service providers to support client success Advocacy & Client Support Serve as a central point of contact for veterans and their families Advocate for clients while helping them navigate complex systems and services Provide ongoing encouragement, accountability, and crisis support as needed Monitoring & Evaluation Regularly review client progress and assess effectiveness of care plans Monitor changes in client condition, barriers, or emerging needs Adjust support strategies and referrals based on client progress and outcomes Documentation & Compliance Maintain accurate, timely, and confidential case notes and client records Ensure documentation meets organizational and program requirements Assist with data tracking, reporting, and outcome measurement Qualifications Bachelor's degree in social work, Human Services, Psychology, Counseling, or a related field is preferred. Relevant experience in case management, social services, nonprofit work, or veteran services may substitute for formal education. Strong communication, organizational, and interpersonal skills. Ability to work independently while also collaborating with a team. Experience working with vulnerable populations, homelessness, mental health, substance use recovery, or veterans is preferred. Proficiency in basic computer systems and documentation platforms. Valid driver's license and reliable transportation. Must currently hold, or be able to obtain, Oklahoma ODHSAS Case Management Level I or II certification. Preferred Qualities Passion for serving veterans and vulnerable populations Strong problem-solving and advocacy skills Ability to maintain professionalism and compassion in high-stress situations Flexible, adaptable, and self-motivated

$20.00 - $27.00 per hour
