This role is ideal for mission‑driven professionals who thrive on direct service, problem‑solving, and community impact. ⭐ About the Role The SSVF Housing Case Manager plays a vital role in supporting veterans and their families as they work toward housing stability and long‑term independence. This position manages a caseload of 20-50 participants , providing individualized, client-centered case management that promotes permanent, affordable housing solutions. The ideal candidate uses critical thinking, independent judgment, and strong problem‑solving skills to assess needs, coordinate resources, and advocate for participants throughout their housing journey. What You'll Do Case Management & Participant Support Apply critical thinking and sound judgment to assess each participant's unique circumstances and determine appropriate service strategies. Develop individualized Housing Stability Plans tailored to participant needs, goals, and barriers. Evaluate complex housing and service barriers and recommend effective interventions to support successful housing outcomes. Maintain regular participant contact through frequent check‑ins to assess progress and engagement. Provide client-centered case management that promotes autonomy, accountability, and long‑term stability. Adjust service approaches as needed based on participant progress or changing circumstances. Collaboration & Advocacy Collaborate with internal teams and external partners. Advocate on behalf of participants with landlords, property managers, and service agencies. Participate in case conferencing, team meetings, and community coordination meetings as required. Attend all mandatory training and team meetings. Documentation & Compliance Ensure case files are complete, accurate, and maintained according to

, and agency standards. Complete and submit required documentation in a timely manner. Prioritize timely, detailed documentation to support continuity of care and program compliance. Work independently to resolve complex barriers while upholding program guidelines and best practices. Or , an equivalent combination of education and relevant work experience. Minimum 2 years of professional experience in social services or a related field required.
