Responsibilities Include:
✔ Helping individuals develop and achieve recovery goals ✔ Coordinating housing, healthcare, employment, educational, and community resources ✔ Conducting substance use screenings and supporting recovery planning ✔ Utilizing CADC expertise to promote treatment engagement and relapse prevention ✔ Connecting participants with treatment providers and recovery supports ✔ Advocating for participants and helping remove barriers to success ✔ Providing services in homes and throughout the community ✔ Transporting participants to appointments and community activities when needed ✔ Completing documentation, assessments, and service plans ✔ Collaborating with treatment providers, families, and community partners Required Qualifications Bachelor's Degree in Human Services or a Related Field Current Certified Alcohol and Drug Counselor (CADC) Certification Valid Driver's License Strong computer and documentation skills Excellent communication and interpersonal skills Minimum two years of experience supporting individuals in: Behavioral Health Addiction Recovery Homeless Services BHH Programs Human Services Settings Preferred Experience ACT or Community-Based Mental Health Services Case Management Experience Experience Supporting Individuals with Serious Mental Illness Knowledge of Recovery-Oriented Practices Familiarity with
