You'll support multiple programs and departments, providing critical case management services wherever they're needed most. Responsibilities Conduct client intake, assessments, and eligibility screenings Develop and monitor individualized service plans Maintain regular client contact, advocacy, and follow-up Connect clients with community resources and support services Maintain accurate documentation, case notes, and electronic records Assist with referrals and service coordination Support multiple departments based on operational needs Provide temporary coverage for various programs throughout the agency Ensure compliance with funding and documentation requirements Qualifications Bachelor's degree required (Human Services, Social Work, Psychology, Sociology, Counseling, or related field preferred) Three or more years of case management or human services experience preferred Strong communication, organization, and documentation skills Experience working with diverse populations facing economic hardship, housing instability, or other barriers HMIS experience is a plus Ability to work independently while collaborating with multidisciplinary teams Important Requirements Reliable transportation is required. This is a fully in-person position. Travel between agency locations throughout Delaware County, including Boothwyn, Chester, Darby, Upper Darby, and surrounding areas, is required.
