Services include intake, referral, outpatient therapy, psychiatric assessment, service coordination, substance abuse treatment, training and social rehabilitative services, crisis intervention, partial hospitalization, community outreach, day, and recreational services for adults with intellectual disabilities, homeless outreach, and housing program. As a Admission/Discharge Case Manager with Chartiers Center, you would be utilizing your exceptional customer service skills and compassionate approach in our behavioral health diversion program managing admissions, referrals/linkages and discharges.
Job duties include:
Serve as the primary liaison for referrals to and discharges from the program. Maintain ongoing and positive communication with referral sources such as psychiatric inpatient units, crisis units and community housing programs. Participate in the Treatment Team to determine consumer needs and service planning/ implementation Coordinate referral for various benefits and services such as Service Coordination, Housing, Mobile Psychiatric Rehabilitation, Peer Support, Mobile Medication, Outpatients Services upon discharge. Minimum Qualifications:
