The Enhanced Case Manager will provide case management services by coordinating benefits, treatment and support resources for participants. As an Enhanced Case Manager with Chartiers Center the core job duties include: Coordinate and manage the delivery of behavioral health, medical social service and community support services. Assess participant needs and develop individualized, person-centered service plans. Meet with participants on a frequent basis to provide ongoing case management interventions and monitor participant progress. Maintain accurate and timely documentation in compliance with organizational and regulatory requirements. Participate in rotating on-call responsibilities to provide after hours support for Janus program participants. Minimum Qualifications:
