As part of a team with over 30 years of combined experience, youll provide essential support to youth and families, helping them grow in resilience and create a brighter future.

Key Responsibilities:
Provide ongoing support to youth and foster families through regular home visitsDevelop individualized service plans to inspire emotional, intellectual, and social growthRespond to crisis situations and provide immediate assistance when neededMaintain accurate records in our Electronic Health Record systemPlan and attend recruitment events, presenting to community groups and organizationsConduct family interviews and complete home studies for foster and kinship families

Requirements:
