Schedule:

Day Shift:

Monday to Friday

We are seeking a detail-oriented Care Manager / Protective Services Worker to provide assessments and protective services for older adults and individuals receiving in-home services. This role is responsible for investigating reports of abuse, neglect, and financial exploitation, conducting comprehensive assessments, and implementing crisis intervention strategies to reduce risk and ensure client safety. Responsibilities Provide in-home service assessments Investigate reports of abuse, neglect, and financial exploitation Conduct comprehensive assessments Develop and implement crisis intervention and safety plans Perform follow-up monitoring to ensure ongoing safety Coordinate services with community providers Maintain accurate, timely documentation in required timeframes

Qualifications:

Detail-oriented with excellent communication and reasoning skills Strong documentation and computer skills Ability to manage multiple cases and meet deadlines

Valid Driver's License Education Requirements:

Bachelor's degree with a social welfare major; OR Bachelor's degree including 12 credits in sociology, social welfare, psychology, gerontology, or related social sciences AND one year of human services experience; OR Any equivalent combination of experience and training, including 12 credits in the above fields, at the agency's discretion.
