Addiction Counselor / Discharge Planner /
Case Manager Schedule:
Monday-Friday, 8:00 a.m.-4:00 p.m.
Work Arrangement:
Office, Regional Site Visits Office Location:
Pittsburgh, PA working from the Johnstown, PA office at least once a month
Education Requirement:
Bachelor's degree in Social Work, Human Services, or a related Human Services field
Pay:
$35-36, per hour Position Summary Provides social work, counseling, case management, and Medication-Assisted Treatment (MAT) coordination services to individuals transitioning from SCI facilities into the community. The position supports continuity of care by coordinating MAT services, treatment referrals, community resources, and individualized treatment plans. The MAT Program Specialist works collaboratively with SCI, BCC, Parole, treatment providers, Center Staff, and other multidisciplinary team members to assess individuals' needs, coordinate services, document treatment activities, and support successful community reentry. This is a hybrid position , with responsibilities performed in an office setting and through required site visits throughout Region 3 . Regional Site Visits Site visits may include the following locations: Erie Johnstown Pittsburgh Braddock Pittsburgh Pittsburgh Coalport Essential Duties and Responsibilities Communicate with the SCI MAT Onsite Specialist when reentrants experience an overdose (OD) in the community and are returned to an SCI for detoxification to determine eligibility and appropriateness for MAT. Assist with MAT referrals in the community, including setting up, coordinating, and scheduling appointments for medication and treatment services. Ensure assigned individuals' needs, benefits, and services are obtained in accordance with their individualized treatment plans. Track and regularly update spreadsheets and databases for individuals released from an SCI while receiving MAT. Attend weekly Vivitrol conference calls and other meetings as needed. Document all client contacts, services, referrals, and treatment activities in appropriate systems, including Captor, Sapphire, and other applicable databases. Attend and successfully complete all required trainings. Meet with reentrants who have positive urine drug screens to assess their needs and make appropriate treatment referrals. Complete GPRA assessments as required, including initial, six-month, and discharge assessments. Meet with reentrants at SCI facilities, BCC locations, Parole Offices, treatment providers, and other appropriate locations as necessary. Communicate treatment planning, client needs, referrals, and relevant updates to Center Staff, SCI Staff, Parole Staff, and other members of the treatment team. Follow and adhere to the MAT/AOD Treatment framework, policies, and procedures established by the Department of Corrections (DOC). Implement and provide social work, counseling, and case management services to individuals to enhance social functioning and support more satisfactory social, economic, and physical adjustment. Assess offenders' psychosocial strengths, challenges, needs for social work services, and eligibility for available services and resources. Provide counseling to individuals utilizing appropriate treatment modalities and interventions. Discuss assigned individuals' psychosocial assessments, family histories, treatment needs, and progress during multidisciplinary treatment team meetings. Coordinate with community treatment providers and other service agencies to facilitate continuity of care and access to appropriate services. Participate in multidisciplinary treatment planning and collaborate with other professionals involved in the individual's care. Conduct follow-up with individuals and service providers as necessary to monitor treatment engagement and progress. Maintain accurate, timely, and confidential records in accordance with applicable requirements. Protect the confidentiality of client information and comply with HIPAA and applicable state confidentiality laws. Perform all duties in accordance with applicable laws, rules, regulations, policies, procedures, and professional standards. Perform other related duties and responsibilities as assigned under the general supervision of an administrative supervisor.
