Program Manager Position Summary-Mon•Fri. (8am-4:30pm OR 8:30 am-5:00pm) Community Action Agency of Delaware County, Inc. is seeking a full-time Case Manager for our Rapid Re-Housing Program. This position is responsible for managing a caseload of homeless families and/or single adults who are working toward securing and maintaining stable housing. The Case Manager will provide intensive case management, supportive services, housing stabilization support, and resource coordination to individuals and families facing multiple barriers. This role uses a Housing First , Progressive Engagement , trauma-informed, and strengths-based approach to help participants achieve housing stability, financial stability, and long-term self-sufficiency. Primary Responsibilities Manage a caseload of approximately 12-15 families and/or single adults participating in the Rapid Re-Housing Program. Complete client intake, assessment, and documentation of financial and program eligibility. Conduct formal inspections of client residences in accordance with HUD Housing Quality Standards. Maintain regular contact with landlords to support lease compliance and housing stability. Develop and maintain individualized housing stability plans with clients and update plans monthly. Assist clients with identifying goals related to housing, income, employment, benefits, budgeting, and self-sufficiency. Connect clients to appropriate community resources, social services, and support systems. Provide advocacy, support, and follow-up to help clients address housing barriers. Provide instruction and support in basic life skills, including budgeting, home maintenance, communication, nutrition, parenting, health/hygiene, and other areas as needed. Monitor client progress through scheduled home visits, office visits, phone calls, and post-exit follow-up. Maintain accurate, detailed, and timely case notes and service documentation. Update housing stability plans and client records as required. Complete required reports for funding sources, monitoring agencies, and agency leadership. Participate in staff meetings, case conferences, monthly recertification meetings, and required trainings. Perform other related duties as assigned. P osition Objectives The Case Manager is expected to: Provide high-quality supportive services that help clients achieve housing stability and self-sufficiency. Develop and maintain a strong network of community resources. Serve as a professional and positive role model for clients. Maintain accurate, complete, and up-to-date client files. Support clients in overcoming barriers to housing and long-term stability. Qualifications Bachelor's degree in Human Services, Social Work, Psychology, Sociology, Counseling, or a related field required. Master's degree in a Human Services field preferred. Experience working with individuals or families experiencing homelessness preferred. Knowledge of Housing First and Progressive Engagement principles preferred. Experience using trauma-informed and strengths-based approaches preferred. Knowledge of Delaware County social service resources preferred. Strong organizational and time-management skills. Ability to manage multiple priorities, deadlines, and client needs. Excellent verbal and written communication skills. Strong interpersonal skills and ability to work effectively with individuals from diverse cultural and socio-economic backgrounds. Proficiency with Microsoft Word and Excel required. Willingness and ability to learn required database systems, including CoC/HMIS-related systems. Driver Requirement This position requires frequent travel throughout Delaware County. Applicants must have a valid driver's license, reliable transportation, and current automobile insurance. Physical Requirements The physical demands described below are representative of those required to successfully perform the essential functions of this position. While performing the duties of this position, the employee is regularly required to talk, hear, stand, walk, sit, climb stairs, stoop, and kneel. The employee must occasionally lift and/or move up to 15 pounds. Specific vision abilities required include close vision, distance vision, peripheral vision, depth perception, and the ability to adjust focus. Work Location and Environment The primary work location for this position is: 1414 Meetinghouse Road Boothwyn, PA 19061 This position requires frequent travel to other agency worksites and client residences throughout Delaware County. The employee may occasionally be exposed to wet and/or humid conditions and outside weather conditions. The noise level in the work environment is generally moderate.
