The ideal candidate will play a crucial role in supporting individuals and families by providing guidance, resources, and advocacy. As a Case Manager, you will work closely with clients to assess their needs, develop personalized plans, and connect them with appropriate services. This position requires strong interpersonal skills, empathy, and a commitment to helping others achieve their goals. Responsibilities Conduct comprehensive assessments to identify client needs and challenges. Develop individualized service plans in collaboration with clients. Provide crisis intervention and support during critical situations. Facilitate access to community resources, including Section 8 housing assistance and workforce development programs. Offer mentoring and support to clients in navigating social services. Monitor client progress and adjust service plans as necessary. Maintain accurate documentation of client interactions and case progress. Collaborate with multidisciplinary teams to ensure holistic care for clients. Provide supervision and guidance to interns or junior staff as needed. ExperienceBACHELOR'S DEGREE

A background in social work or related fields is preferred. Experience in crisis management OR behavioral health highly desirable. Familiarity with motivational interviewing techniques is a plus.
