Case Manager I DCFCM • Diakon Youth Services York, PA "At Risk" Youth Services Per Diem, Varied
Hours/Pay Period:
0-29 Req # 10274 Diakon is one of the premier employers in Pennsylvania and Maryland. We are a non-profit organization which includes Diakon Child, Family, and Community Ministries and Diakon Affordable Housing.
General Responsibilities:
Responsible for serving as a resource person, case manager, counselor and trainer to support the Bridge Program. Meets regularly in youths' home to provide skill development and on-going case management services to coordinate the treatment planning and services provided according to the Treatment Plan and clinical direction. Establishes and maintains professional and supportive relationships with youth, birth families, referring personnel, treatment providers, and others involved in the lives of children and youth Maintains regular contact with referring personnel, and attends and testifies at court proceedings involving children and youth in care.
Education/Training:
A Bachelors degree in social service or a related field of study normally required.
Experience:
A minimum of one (1) year's experience in a related child welfare or human service position normally required.
ADA:
Ability to sit, stand, lift, bend, reach, push, and pull items weighing up to 50 lbs.; visual and auditory skills.
