A Care Management Agency is seeking Bilingual Care Managers to join our growing team! Care Managers provide services to children who meet the eligibility requirements for the NYS Children's Health Home Program. The main role of the Care Manager is care coordination and connecting children and families to appropriate services and supports. This position does not involve providing hands-on home care or personal care services. Full Time This role follows a hybrid work model, requiring a combination of fieldwork and office-based responsibilities.

Responsibilities:

Participate in initial and ongoing training to maintain and enhance care management skills. Provide care coordination services for children. Conduct required monthly in-person meetings with clients. Identify the needs of each child and coordinate services relevant to the Plan of Care. Maintain communication with families, providers, and community supports. Document all case activities in a timely and accurate manner.

Qualifications:

Bachelor's degree with 2 years of relevant experience or Masters degree with 1 year of experience. Bilingual in Spanish, Hindi, Bengali, or Urdu Solid writing and verbal communication skills.

Pay:

$24.00 - $26.00 per hour
