Case Management Coordinator II•Temporary Assignment Montebello, CA | Hybrid Schedule | $23•26/hr We are seeking a Case Management Coordinator II for a temporary assignment with a managed care organization based in Montebello, CA. This is an excellent opportunity for candidates with prior authorization, care coordination, and managed care experience who are passionate about supporting patient care and connecting members to healthcare resources.

Monday-Friday | 8:00 AM•5:00 PM Hybrid Schedule•onsite 1x per week
Pay Range:
$23•26/hr Qualifications High school diploma required Minimum 2 years of healthcare experience Experience working within managed care, IPA, HMO, or health plan environments Knowledge of prior authorization and case management processes Bilingual English/Spanish preferred Medical Billing Certification strongly preferred Medical Assistant experience strongly preferred Key Responsibilities Process and coordinate case management and care coordination activities Verify member eligibility and benefits Utilize DOFRs (Division of Financial Responsibility) and delegation agreements to support care coordination decisions Coordinate appointments, transportation, referrals, and community resources for members Conduct patient outreach to ensure care needs and services are being met Collaborate with Case Managers, providers, specialists, and interdisciplinary teams Gather patient information for assessments, care planning, and transitions of care Maintain accurate documentation within the case management system Support departmental productivity, audit, and model of care goals This role is ideal for someone who enjoys working in a fast-paced managed care environment while making a meaningful impact on patient care.
Pay:
$23.00•$26.00 per hour
