Sumter SC, Florence SC About Us Serenity Home Health Care, LLC is a compassionate and community-focused provider of case management services across South Carolina. We specialize in guiding individuals and families through long-term care options under the Guidelines of our State and Federal Agencies as well as Community-Based Services (HCBS) Waiver Program. Our mission is to ensure participants receive the support and services they need to thrive in their communities. Position Summary We are seeking a dedicated and organized Case Manager to join our team. This role is rooted in social work and involves coordinating services, conducting assessments, and supporting participants in accessing medical, social, and community-based resources. The Case Manager serves as a vital link between participants and the services they need. Key Responsibilities Conduct monthly participant contact, including face-to-face visits. Develop, implement, and monitor person-centered service plans. Coordinate and authorize waiver services, including changes and terminations. Perform ongoing assessments, evaluations, and re-evaluations. Maintain accurate documentation using the Phoenix and EVV systems. Collaborate with community resources and participate in interagency staffing. Ensure compliance with all SCDHHS policies and HCBS program requirements. Travel within assigned regional areas (minimum of one CLTC Regional Office). Maintain participant confidentiality and adhere to mandated reporting requirements. Qualifications Bachelor's degree in Social Work or a related health/human services field. Minimum of 2 years of experience in assessments and care planning. Valid Driver's License and reliable transportation. Clear SLED background check and 10-year Motor Vehicle Record (MVR). TB test, car insurance, and ability to pass a knowledge test. Strong communication, time management, and problem-solving skills. Proficiency in computer systems and electronic documentation. Must be at least 18 years old. Preferred Skills Licensed Social Worker (LBSW or LMSW) in South Carolina. Experience with Medicaid HCBS Waiver programs. Familiarity with the Phoenix system and EVV documentation. Work Conditions Must be available Monday through Friday, 8:30 a.m. to 5:00 p.m. Cannot hold other employment exceeding 4 hours during business hours. Must be able to travel up to 30% or more using a personal vehicle.
