Job requires significant travel within the community, home visits, assessing needs, developing plans, monitoring services, and working with providers.

Qualifications:

Bachelor's degree or higher and have at least one (1) year of experience working with people with disabilities or one (1) year of Case Management experience. OR Registered Nurse licensed in the State of South Carolina and have at least one (1) year of experience working with people with disabilities or one (1) year of Case Management experience. All degrees must be from a college or higher education institution recognized by the US Department of Education/the Council for Higher Education Accreditation (CHEA).
