Case Manager - Veterans Services Program Homeward Bound Adirondacks (HBA) SSG Fox Suicide Prevention Grant Hybrid Position | Albany Area with Travel Throughout the North Country Full-Time Homeward Bound Adirondacks is seeking a Case Manager to join our SSG Fox Suicide Prevention Grant program. This position will provide case management and supportive services to Veterans throughout the Albany area and North Country, with a primary focus on suicide prevention, outreach, engagement, connection to services, and strengthening Veterans' social and community supports. The successful candidate will be compassionate, highly organized, dependable, and comfortable working independently in both community and home-based settings. Position Responsibilities The Case Manager will: Provide individualized case management services to Veterans experiencing challenges related to post-service life. Conduct proactive outreach and engagement with Veterans who may be isolated, underserved, or reluctant to seek traditional services. Conduct home visits and community-based visits as appropriate. Provide or coordinate transportation to VA appointments, medical and behavioral health services, and other essential resources. Assist Veterans with linkage to the VA and other community-based services and resources . Complete referrals and follow-up to ensure Veterans successfully connect with needed services. Assess Veterans' needs and develop individualized service plans focused on safety, stability, wellness, and connection to resources. Complete suicide prevention screenings, assessments, documentation, service plans, progress notes, and required program data entry in a timely and accurate manner. Identify Veterans who may benefit from HBA's Adirondack wilderness retreats and facilitate appropriate referrals and follow-up. Provide ongoing support to Veterans participating in retreats and other HBA programs as appropriate. Assist Veterans with accessing resources related to housing, food insecurity, transportation, financial emergencies, behavioral health, substance use, employment, legal concerns, and other identified needs. Develop positive working relationships with VA staff, Veteran Service Officers, healthcare providers, community agencies, and other Veteran-serving organizations. Participate in and help facilitate community outreach and marketing events designed to increase awareness of Veteran services and suicide prevention resources. Represent Homeward Bound Adirondacks professionally at community events, Veteran gatherings, outreach activities, and partner meetings. Maintain accurate, timely, and confidential client records in accordance with HBA policies, grant requirements, and applicable regulations. Participate in staff meetings, supervision, training, and other program activities. Perform other duties consistent with the goals and objectives of the Veterans Services Program. Travel This is a hybrid, community-based position . The primary geographic focus will be the Albany area , with regular travel to HBA's North Country base camps in Saranac Lake and Malone , as well as travel to retreat locations, Veterans' homes, community events, VA facilities, and other service locations. Regular travel throughout the service area is an essential function of this position. Qualifications The ideal candidate will have: Strong interpersonal and communication skills and the ability to establish trusting relationships with Veterans. The ability to engage individuals who may be isolated, in crisis, or hesitant to access services. Strong organizational skills and exceptional attention to detail. Strong computer skills and the ability to learn and consistently use electronic documentation and data systems. Ability to complete assessments, service plans, progress notes, and other required documentation accurately and within established timelines. Ability to work independently while also functioning effectively as part of a team. Professional judgment, reliability, flexibility, and a commitment to confidentiality. Valid driver's license and reliable transportation. Willingness and ability to travel throughout the program's service area.
