In this vital role, you will be apart the Family Resource Connection and Preservation Services that provides short-term, family focused services designed to promote safety and stable families, support family strength and stablility, and enhance parental functioning, and protect children. This contractual position offers an exciting opportunity to apply your social services skills in a flexible and supportive environment dedicated to excellence in client care. Duties Conduct thorough biopsychosocials and comprehensive care plans to determine individual needs and treatment plans. Develop and implement personalized case management strategies that incorporate parenting skills, access to resources, medical wellness, educational resources, and concrete needs. Utilize Motional Interviewing Skills to promote client motivation to change (training will be provided) Maintain accurate electronic health records (EHR) in compliance with HIPAA regulations, documenting all patient interactions, assessments, and discharge plans meticulously.

Qualifications:

BA in Psychology, Social Work, Human Services

LMSW, BSW
