CMA Coordinator Pay:
$22.32 - $23.69/hour
Schedule:
Hybrid (field + remote) in the Colorado Springs area Position Summary The CMA Coordinator supports individuals in accessing and coordinating long-term care services. This role works closely with individuals, families, and providers to develop person-centered service plans that promote independence, health, and safety. Key Responsibilities Develop and coordinate individualized service plans Serve as a primary contact for individuals, families, and providers Monitor services to ensure quality, compliance, and effectiveness Advocate for individuals and connect them to community resources Maintain accurate documentation in electronic systems Conduct in-person visits in homes, facilities, and community settings Participate in team meetings, trainings, and ongoing development Qualifications Bachelor's degree or 5 years of relevant experience in human services, case management, or working with vulnerable populations Experience with Medicaid waivers or long-term care services preferred Strong communication, organization, and problem-solving skills Comfortable with technology, databases, and Microsoft 365 Requirements Valid driver's license and reliable transportation Clean Motor Vehicle Report, no major moving violations in the past 3 years Ability to travel locally Benefits Health, dental, vision insurance 401(k) with employer contributions Paid time off + holidays Flexible schedule + half-day Fridays Professional development opportunities Mileage reimbursement
